Please note regarding Quebec: Merchant Law Group cannot offer to provide any advice (nor potential assistance) to individuals who are resident in Quebec or received a Depuy hip implant surgery in Quebec. For such individuals, a separate Quebec class action settlement has already been reached regarding the two above-referenced hip implant models, and the deadline for Quebec individuals to apply for compensation is May 24, 2019. Please do not fill out the form below [nor contact our law firm] if you are from Quebec. Instead, please email firstname.lastname@example.org for more information — and read the following settlement notice https://www.newswire.ca/news-releases/notice-of-settlement-approval-683431361.html or visit the Quebec settlement administrator’s website at https://collectiva.ca/en/our-cases/depuy-asr-hip-implants/. To make a compensation claim under the Quebec settlement, you must take steps on your own [or through your personal lawyer], to submit an individual Claim Form before the May 24, 2019 deadline. Please do not submit your claim form or any documentation to our law offices, as we cannot take any steps on your behalf. If you fail to submit a Claim Form by the May 24, 2019 deadline, you will be forever disentitled from making a legal claim to receive compensation.
